Have I suggested a trackball yet? I had to switch to them decades ago (along with ergonomic keyboards) due to carpal tunnel symptoms, and I haven't looked back! Very precise, but because you just rest your hand on them, rather than having to grip them and move them around, you don't wind up moving your arm nearly as much. Learning to use your thumb for pointing takes a little getting used to, but it might be really helpful for your pain levels.

Lol! I've already got this thumb track ball. It does take getting used to. This design could use an extended stock for the last three digits could get a steadier grip. My lateral thumb movement is a bit weak but very doable. My son has located a keyboard that may adapt to my needs.

Wow, yeah, that one does look teeny! The pics below are of the form factor I've been using for a long time. The one I used when I first switched (Logitech Trackman) was broader and flatter, but these newer versions aren't too much more "humped up," so they're still fairly comfy for my hand, and do support my whole hand even though my fingers are a good bit longer than most ladies' fingers. I took a pic of my hand on top of it, but also one upside down, so you could see how much of my hand it supports.

I've used several with similar form factors. The evolution of the OG is the Logictech M570, and I also used an Elecom M-XT2URBK-G wired version for a while. The issue I found with both of those is that the switches under the mouse buttons start mis-clicking after about 2 years of daily use. Either double-clicking when they're not supposed to, or not clicking when you click. I actually went through 4 or 5 of the Logitechs before I started looking for alternatives. The one I would NOT recommend is the Perixx Perimice-717. Similar form factor, but the ball was so stiff in its socket that it was impossible to use with any precision. I think I used it 2 days before giving up in frustration.

If clicking the left button gives you any pain, I probably wouldn't recommend this one I'm using now (Nulea M501), because the clicking action is stiffer than any other I've ever used. It's fine now that I'm acclimated, but if that is painful for you, you'd probably want to avoid it. The others have very easy clicks, and might be worth the shorter lifespan to have a more comfortable click each time. (Not that I know how long the Nulea's switches will last...I've only owned it since January of this year. But I love that it's rechargeable, whereas all the others were either wired or required AA batteries.)

Oh, and the gray thing under my wrist on the desk is a Jelbows gel pad. Absolute necessity for allowing me to work all day comfortably while resting my arm on the desk!

It's just a matter of finding the right combination of devices. I just found a store with a mind numbing array of input devices of various brands. I'll need to set aside a day to visit that store.
