La aftas suelen ser muy comunes en muchos pacientes son conocidas como las famosas yagas que salen en la boca, suelen ser bastante dolorosas porque esta se ulceran y suelen ocasionar en el paciente bastante molestias que en muchos casos limitan la masticación de los alimentos, a consulta odontológica asistió paciente refiriendo en el en su motivo de consulta llagas de gran tamaño numerosas en los carrillos, cara dorsal de la lengua, así como también labio inferior interno, nos llamó bastante la atención que la paciente expresó que salieron justamente dos días después que asistió hace una semana atrás a la consulta odontológica, por lo que ya tenía más de una semana de evolución.

Las aftas no representan malignidad en la cavidad bucal, forman parte de las patologías del tejido epitelial, es la lesión más común que suele aparecer en la cavidad oral debido a que estas úlceras son dolorosas y las observamos en la mucosa, ya que carecen poca queratinización, la localización más común que tiene es en la mucosa de los labios, carrillos así como también en el vientre de la lengua, en el paladar hablando y en el borde del bermellón de los labios, conjuntamente también puede salir en los pilares del istmo de las fauces, en el piso de la boca, cabe destacar que solo aparece en cavidad oral, nunca van a aparecer en el paladar duro y en la piel.

Debemos conocer cada una de los tipos de actas que existen para poder determinar cuál es la presencia del tipo de afta a la que nos estamos enfrentando, las actas menores suelen ser muy comunes en la infancia suele manifestarse en pacientes de 8 a 10 años, pero también podemos encontrarlas en pacientes adultos estas tendrán un tamaño bastante pequeño que será aproximadamente de 0 a 3 mm, con una profundidad de 5 mm cuando hablamos de actas menores esta van a tener una duración de 7 a 14 días, pueden tener una recurrencia de repetición a nivel de la mucosa bucal de una a cinco actas, cabe resaltar que es muy importante tener en cuenta que son de pequeño tamaño por eso son aftas menores.

Ahora bien hablaré de las aftas mayores, que es el tipo de lesiones que presenta nuestra paciente a la que hago mención estas son más grandes de mayor tamaño y suelen durar aproximadamente seis semanas, a pesar que nuestra paciente solo tiene una semana de evolución se puede diferenciar que son actas mayores debido a la gran extensión que tiene en la cavidad bucal, es decir no se encuentra en un solo lugar en específico sino que también están en la lengua, en el piso de boca así como también en las mejillas el tamaño que posee va desde los 5 mm a un centímetro y suelen aparecer de manera repetida más de 10 veces, en la cavidad bucal, en nuestra paciente podemos observar que posee aftas de gran tamaño qué tienen un espesor de más de 1.5 mm de profundidad, esta aparecerá en paciente desde los 12 años en adelante en nuestra paciente apareció justamente a los 24 años.

Por último están las las herpetiformes, que se encuentran en paciente que poseen más de 40 años, en adelante estas suelen ser en formas de racimos de úlceras recurrentes y múltiples, que clínicamente las podemos evidenciar con ciertas características similares, al igual que las lesiones por herpes pero no se encuentran relacionadas a ningún virus en específico, estas suelen producir fiebre y diversas adenopatías que ocasionan gran dolor son lesiones que se unen entre sí y forman una lesión mucho más amplia.

Para finalizar debemos asistir a consulta odontológica, para poder recibir tratamiento en caso de manifestar lesiones como la que presenta nuestra paciente el tratamiento idóneo son los antiácidos ya sea en gel o en suspensión, así como también las cápsulas de vitamina E, aplicando su líquido en el interior de la zona lesionada es decir en dónde se encuentra la lesión, cabe destacar que para lograr la correcta desaparición de las estomastitis aftosa se debe lograr remover la delgada capa bacteriana, que posee cada una de las lesiones posteriormente que se logra cumplir el tratamiento se debe aplicar enjuague bucal sin alcohol lo que es más conveniente usar enjuagues de clorhexidina.

En nuestra paciente esta estomatitis aftosa mayor, fue producida justamente por un instrumento que no se encontraba bien esterilizado, ya que inmediatamente después de la consulta odontológica aparecieron las lesiones, por eso es muy importante que cada uno del instrumental y material odontológico, se encuentre debidamente esterilizado de manera adecuada.
