In one of my previous posts, I talked about the importance of clinical diagnosis. Even though the initial ECG was normal, due to my strong clinical suspicion, I went for a second, a specific blood test, and my suspicion came true for an acute heart attack. Today, I am going to share another short story where my clinical suspicion was incorrect. A relevant investigation was quite useful in that regard!
A teenager came with abdominal pain. He also vomited for a few episodes. There was no fever, no urinary problem, and no diarrhea. On examination, I found significant tenderness in the right lower abdominal area. Examination didn't elicit tenderness in other areas. Considering the location of the tenderness, my provisional diagnosis was a case of acute appendicitis. I referred him to the higher center!
Later, I called his father to learn about the final diagnosis. They went to the higher center and did an ultrasound scan. The test didn't reveal the features of an acute appendicitis. The doctor labelled it as a case of dyspepsia/indigestion. An investigation helped the doctor confirm a diagnosis.
Often, poor patients hesitate to do the investigations and ask for medicines only! We give the management according to the scenario, without the relevant investigations. But investigations help us to prescribe specific medicines!

My Saturday was pretty busy at work, especially the evening shift. In total, I consulted 40 patients in my chamber, two-thirds of them visited in the evening shift. A good number of school students came with seasonal flu symptoms.
At home, I had a second round of morning sleep after the Fajr prayer. In the afternoon, I skipped the afternoon nap. I was able to walk on the treadmill for 10 minutes. According to the Fitbit app, I made 10,207 steps on Saturday!
