Three GP doctors were on duty on Wednesday. Hence, it was a pretty relaxing duty for me. I consulted only 21 patients in my chamber. Of them, there was a regular patient of mine. She is hypothyroid and comes every 3 months for follow-up. In my chamber, whenever I check her BP, I find it quite high. After resting for half an hour, one of the nurses checked her BP outside my chamber. It was still high. However, at home, with the digital BP machine, her BP remains within the normal range. Moreover, she didn't have any symptoms of raised BP. Hence, looked like it was most likely a case of white coat hypertension!
Another expatriate patient came with his sponsor. The sponsor managed health insurance for him. But, every step to get "health service", his policy required pre-approval from the insurance company! He came with upper abdominal pain for over a month. I wanted to do H. Pylori test. When we applied for approval, it was rejected. The test wasn't even in the list of tests that would cover his premium. Such a low-quality premium he had!! Even though he had health insurance, his sponsor needed to pay in cash!
At home, I couldn't walk before or after lunch. I could walk only after 10 pm, and I walked for 15 minutes. It helped me cross the 10k steps mark!

Are you suffering from appendicitis?
Acute appendicitis is one of the commonest causes of severe acute abdominal pain. Young people, mostly teenagers, commonly suffer from this condition. If not treated timely, some patients may present with complications. A small portion of the appendicitis cases may present way later as chronic appendicitis. Prompt diagnosis and early intervention are crucial for acute appendicitis.
The Cleveland Clinic listed a few signs and symptoms of appendicitis in a short video. Let us go through the list quickly! You will find the video at the end of this short post.
Acute abdominal pain that starts around the umbilicus. It later shifts to the right lower abdominal area.
Vomiting tendency. Some patients may vomit as well.
Fever; not every patient, but a good number of the patients having appendicitis may have fever.
Lack of appetite, Malaise, and fatigue (usually indicate chronic appendicitis)
Swelling and rigidity of the abdomen; it indicates the possibility of the complication called “burst appendix”
Increased urine output and altered bowel habits (either diarrhea or constipation)
Examining the patient by an experienced surgeon, doing some lab tests, and performing an ultrasound scan can confirm or rule out appendicitis!

It's unfortunate how some insurance policies can be so restrictive, especially when it comes to essential diagnostic tests like H. Pylori.
It must have been a tough situation to navigate, especially with the sponsor having to step in and pay out of pocket despite the patient being insured.
Hopefully, they can reconsider their policy options in the future to avoid such issues.

My doctor, trying to wean me from the fentanyl patch, prescribed a lower dosage. The pharmacy called and said my insurance will not cover it, they cover the higher dosage. I find this crazy.
