Stroke begins the instant blood flow to part of the brain suddenly stops. Brain cells depend on a constant supply of oxygen to produce energy, without these fresh oxygen, these cells begin to fail within seconds. The first change is a rapid drop in the energy the cells use to keep their membranes stable.
When this energy disappears, the delicate balance of salt and water inside each cell becomes disrupted, causing sodium and calcium to begin rush inward while potassium leaks outward. This shift causes the cells to swell and lose their ability to send normal electrical signals.
As the imbalance grows, the cells release large amounts of glutamate. "Glutamate is a chemical messenger that normally helps in communication", but in this moment it becomes harmful because it have overstimulate nearby the cells.
These neighboring cells also pull in too much calcium, which damages important structures inside them. Enzymes become activated at the wrong time and begin breaking down proteins and membranes.
Within the first minute, the affected area of the brain becomes silent because the cells can no longer function. Although they are not dead yet, they are in a fragile state where continued oxygen loss will cause permanent injury. This early window is the beginning of the stroke process.
